  • One overall rating. "How satisfied were you with X?" on a 1 to 5 scale. This becomes the metric you watch over time.
  • One specific rating. Rate the part you are trying to improve right now: speed, clarity, friendliness, value for money.
  • What worked. "What did you like most?" tells you what to protect.
  • What to improve. "If you could change one thing, what would it be?" The single most actionable question on any feedback form.
  • Follow-up permission. An optional email field so you can close the loop with unhappy respondents.
